Official Rules: 2017 “Summer Drive to Save Lives” Raffle
Official Rules – The raffle is sponsored by the Maricopa County Valley Volkswagen dealers and United Blood Services’ Arizona Operations (UBS/AZ), a non-profit subsidiary of Blood Systems.
Eligibility – The raffle is open to United States residents, who are at least 16 years of age on the date of United Blood Services’ Volkswagen grand prize ceremony, who present to give blood at any United Blood Services’ donor center or mobile blood drive operated out of Arizona and for non-donors who visit the United Blood Services headquarters located in Scottsdale during the raffle timeframe. In the event the winner is younger than 18 years of age on the date of United Blood Services Volkswagen grand prize ceremony, the prize will be awarded to the legal parent/guardian of the finalist.
Employees of United Blood Services, Blood Systems, Volkswagen or their subsidiaries and their immediate family members are not eligible to enter. The term “immediate family” includes spouses, siblings, parents, children, grandparents, grandchildren or other persons residing in the same household (related or not). The raffle is void where prohibited. Residents of jurisdictions where this contest is not permitted, or where United Blood Services would be required to register or be restricted in any other way, are not eligible to win a prize.
How to Enter – Persons presenting at a participating UBS/AZ location from June 1, 2017, through August 31, 2017, are eligible to enter the raffle.
First Entry: Persons intending to donate blood must complete a donor eligibility interview to enter the raffle. Persons who have completed the donor eligibility interview will be automatically entered into the raffle and do not need to complete an official entry. It is the responsibility of the person intending to donate to ensure that UBS/AZ has the correct contact phone number in the event their name is drawn as a finalist. Persons who do not wish to be automatically entered into the contest may call 480-675-5454 and leave their name, phone number and date of donor eligibility interview to be removed from the drawing. Only one donor eligibility interview is permitted per person per day.
Non-Donor Entry: No purchase or blood donation is required to enter the raffle or to win a prize. Persons not intending to donate blood (non-donors) must obtain and submit an official Bonus Entry Card in person from the UBS/AZ administrative office main reception desk located at 6220 E. Oak Street in Scottsdale to enter the raffle. Entries at the UBS/AZ administrative office will be accepted only Monday through Friday, holidays excluded, between the hours of 8:00 a.m. and 4:00 p.m. Bonus Entry Cards sent through the U.S. mail, by fax or by email will not be accepted. The Bonus Entry Card is non-transferable. Completed Bonus Entry Cards that are illegible, inaccurate or that have been reproduced will be disqualified. Only one Bonus Entry Card is allowed per person per day. All non-donor Bonus Entry Cards must be submitted at the UBS/AZ administrative office main reception desk during the designated hours/days no later than August 31, 2017.
Bonus Entries 1 & 2: All persons completing the donor eligibility interview will be offered one Bonus Entry Card to get a second and third chance to enter the raffle. The entrant will be allowed to register the Bonus Entry Card online (Bonus 1), deposit the Bonus Entry Card (Bonus 2) at a designated Arizona Volkswagen dealer, or both, according to the rules and timelines described below. The Bonus Entry Card is non-transferable. Completed Bonus Entry Cards that are illegible, inaccurate or that have been reproduced will be disqualified. Bonus Entry Cards that are mailed or faxed to a Volkswagen dealer, UBS/AZ or UBS/AZ donation location are not eligible.
- Bonus Entry 1 – Online: To obtain a second chance to win, the entrant must register their Bonus Entry Card at www.ArizonaHero.org/VW. Each Bonus Entry Card with its unique code may only be registered once. Only one Bonus Entry Card may be registered per person per day. All online Bonus Entry Cards must be registered by 11:59 p.m. on September 4, 2017. In the event the number of codes registered exceeds the number of pre-donation interviews, the entrant will be disqualified.
- Bonus Entry 2 – VW Raffle Box: To obtain the third chance to win, the entrant must visit any of the designated Volkswagen dealers to deposit the Bonus Entry Card in the official entry raffle box. The Bonus Entry Card may be deposited in the raffle box by the entrant or given to a Volkswagen employee on-site at any of the participating dealer facilities for deposit. Persons are not required to meet with a Volkswagen sales representative or to make a purchase at a Volkswagen dealer to be eligible to deposit an entry. Only one Bonus Entry Card at a Valley Volkswagen dealer is allowed per person per day. All Bonus Entry Cards must be deposited by 5:00 p.m. on September 4, 2017, at one of the designated Volkswagen dealers: Larry Miller Volkswagen (Avondale), Berge Volkswagen (Gilbert), San Tan Volkswagen (Gilbert), Lunde’s Peoria Volkswagen (Peoria), Camelback Volkswagen (Phoenix), Volkswagen North Scottsdale (Phoenix), Chapman Volkswagen (Scottsdale), Findlay Volkswagen (Flagstaff) or Larry H. Miller Volkswagen (Tucson). In the event the number of Bonus Entry Cards deposited exceeds the number of pre-donation interviews, the entrant will be disqualified.
Prize – A maximum prize of one 2017 Volkswagen Passat R-Line valued at approximately $25,245 MSRP will be awarded. The winner may upgrade the prize vehicle at the discretion of Volkswagen and at the winner’s expense, as long as the prize vehicle awarded is a Volkswagen model. The substitution or upgrade to another vehicle make/manufacturer is not permitted. The winner of the prize is responsible for all taxes, license, registration fees and any other miscellaneous expenses.
Selection of Finalists and the Grand Prize Winner – The raffle entry period starts on June 1, 2017, and ends on August 31, 2017. Ten finalists’ names will be drawn based on the criteria below. While donors are eligible to be entered into multiple finalist drawings, only one of the ten spots can be garnered by each finalist. In the event the same donor is drawn for a second finalist spot, an alternate name will be drawn.
Three Finalists: One finalist’s name will be drawn within five days after the end of each of the following date ranges from among those who presented to give blood at a UBS/AZ mobile blood drive or donor center, excluding participants from the July 2, 2017, Saving Arizona blood drive special event:
- June 1 – 30: UBS/AZ blood drive & center donors
- July 1 – 31: UBS/AZ blood drive & center donors (July 2 Saving Arizona event donors excluded)
- August 1 – 31: UBS/AZ blood drive & center donors
One Special Event Finalist: One finalist’s name will be drawn within five days following the event from among the July 2, 2017, Saving Arizona blood drive special event donors.
One MAX Donor Finalist: In addition to being included in drawings 1 – 3, one finalist’s name will be drawn within five days after the end of the raffle from among those who complete a MAX donation (specific automated donation procedure based on blood type) at a UBS/AZ Valley donor center June 1 – August 31.
One Blood Drive Power Donor Finalist: In addition to being included in drawings 1 – 3, one finalist’s name will be drawn within five days after the end of the raffle from among those who complete a Power Donation (Power Red, Red Cell/Plasma or Double Plasma) at a UBS/AZ blood drive June 1 – August 31.
One Teen Finalist: In addition to being included in drawings 1 – 3, one finalist’s name will be drawn within five days after the end of the raffle from among 16- to 19-year-old donors who meet all three of the following requirements:
- Completes the online pre-registration at www.AZHero.org/teencarfinalist
- Is between the ages of 16 and 19 years old during the timeframe of June 1 – August 31, 2017
- Presents to donate at a UBS/AZ blood drive or donor center June 1 – August 31, 2017
One Goal-Making Blood Drive Coordinator Finalist: One finalist’s name will be drawn within five days after the end of the raffle from among the primary blood drive coordinators who achieved or exceeded their official blood drive goal June 1 – August 31. Only one person may be designated as the primary blood drive coordinator for each organization hosting a blood drive. One entry may be earned for each goal-making blood drive operation that requires a separate UBS/AZ Donor Services’ staff change.
One Bonus Entry Card 1 Finalist: One finalist’s name will be drawn within ten days after the online registration deadline from among the Bonus Entry Cards registered at www.ArizonaHero.org/VW, combined with any qualified non-donor entries received during the campaign months.
One Bonus Entry Card 2 Finalist: One finalist’s name will be drawn within ten days after the raffle box deposit deadline from among the combined Bonus Entry Cards deposited in the official entry boxes at the designated Volkswagen dealers.
United Blood Services will conduct an audit to verify the names drawn as the two Bonus Entry Finalists from among the Bonus Entry Cards registered online and deposited at designated locations were submitted in compliance with the raffle rules and will draw an alternate name(s) in the event of any fraud or deception.
UBS/AZ will use its reasonable best efforts to contact each finalist by telephone for two business days following the drawing. If contact cannot be made, the finalist will be deemed ineligible and an alternate name will be randomly drawn from among the remaining entries on the third business day. Drawings will be managed by the Donor Recruitment Marketing Team at the UBS/AZ headquarters located at 6220 E. Oak Street in Scottsdale, Arizona, and will be held under the supervision of two United Blood Services employees, whose decisions are final and binding.
Each finalist will be required to provide proof of a driver’s license or learner’s permit valid in the United States and proof of insurance and sign the United Blood Services Affidavit of Eligibility statement to participate in the grand prize ceremony. Through the Affidavit of Eligibility statement, each finalist must agree to accept responsibility for all taxes, license fees, registration fees and any other miscellaneous expenses in the event he/she wins the grand prize drawing. In the event the finalist is younger than 18 years of age on the date the Affidavit of Eligibility is signed, the finalist’s parent/legal guardian must also sign the Affidavit of Eligibility and provide proof of a driver’s license valid in the United States and proof of insurance. By signing the Affidavit of Eligibility, all finalists also grant UBS/AZ and Volkswagen permission to use their names, likeness, photographs and voices in connection with promotion of the raffle and waive any claims to royalty, right or remuneration for such use. The Affidavit of Eligibility must be completed and received UBS/AZ within five business days of receipt by the finalist in order for the finalist to be eligible for the grand prize ceremony. If the Affidavit of Eligibility is returned as undeliverable, or is not received by UBS/AZ within the five business days as required, the finalist will be disqualified and an alternate name will be drawn. Non-compliance with any of the foregoing may result in disqualification and the drawing of an alternate finalist name. UBS/AZ reserves the right to permanently disqualify any person who it believes has intentionally violated the Official Rules.
The ten finalists will be invited to the grand prize ceremony that will be held on or before October 6, 2017. Finalists must be present at the grand prize ceremony in order to be eligible to win the prize. One of the finalists will draw the key that will start the engine of a 2017 Volkswagen, and the keys drawn by the other finalists will not start an engine. The finalist whose key starts the automobile’s engine will be deemed the contest winner and will be awarded a 2017 Volkswagen Passat R-Line. The identifying VIN of the vehicle with the winning key will be placed in a sealed envelope by the designated Volkswagen representative. Only one 2017 Passat R-Line will be awarded. In the event of a malfunction that causes the engine of more than one vehicle, or no vehicle, to be started by a grand prize ceremony key, the finalist holding the key of the vehicle identified in the sealed envelope will be deemed the contest winner.
Miscellaneous – The odds of winning depend upon the number of eligible entries received according to the eligibility requirements. By entering, participants agree to be bound by these Official Rules and decisions of UBS/AZ. By participating in the drawing, finalists agree to release and hold Blood Systems, United Blood Services, Volkswagen and their employees harmless from any and all losses, damages, rights, claims and actions of any kind in connection with the drawing or the raffle, or resulting from acceptance, possession or use of any prize, including without limitation, personal injuries, death and property damage, and claims based on publicity rights, defamation or invasions of privacy. UBS/AZ reserves the right to change these Official Rules without notice.
The conduct of this raffle is governed by the applicable United States laws, which take precedence over any rule to the contrary herein. United Blood Services will follow the applicable laws for conducting raffle.
